I am wanting to set the subscription types for contacts when they fill out certain forms, how do we do this?

You can do this directly in the forms editor. Click on the consent notice text, the left sidebar should open and this is where you can select and adjust the subscription checkboxes:
If you’re not seeing this, click into the empty gray area of the forms editor and your sidebar should look like below. You can then use the dropdown to display the subscription checkbox:
