This is something that we see happen when a contact fills out a form for multiple people on the same computer. When the first form submission is entered, the browser cookie links that web browser to the contact with the email address that was submitted. If that computer is used to fill out the form multiple times for multiple people, because that browser is assigned to the first of the contacts, the form submission will continue showing up on that contact record, instead of creating new contacts.
If you have employees filling out a form for your customers, this would be common.
There’s an easy solution to prevent this from happening again. In your form settings on the form, you’ll see at the bottom of the settings "Always create contact for new email address" If you toggle that, any time that form is used with a new email address, it’ll create a new contact instead of adding it to the contact associated with that browser cookie.
