i met some wired situation recently, i created a form and insert the form to email so that personl who received the email can fill out the form. but unfortunately, some form submission didn’t associate to a contact. there are 2 situations:

@Jaylin the email field is required for reliable association with a contact record.
Any previous association happened through cookies and was in that sense luck. The moment a user clears his cookies, uses privacy extensions or a privacy focused browser, a different device or browser altogether, HubSpot cannot make the connection.
Since none of the above can be ruled out, the email field must be on the form. You can prepopulate it but it must be there.
@Jaylin you can’t, as far as I know. You would have to manually enter whatever information was submitted on the contact record but it’s not possible to actually link submission and contact after the fact.
