For several weeks our form submission date has been off by months. How do I correct this?

Are you referring to July 12 or Dec 30?
The July 12 is simply part of the name of the form, it doesn’t have to do with the submission itself. Click on the first hyperlink and you can go into the forms editor to clean up the form name

honest mistake there!
I’d suggest to you editing the name of your forms
to help you understand better what is does
for example: web | New leads or Event | MQLs
you can always access you forms and edit the name on the top with the pencil.
