Root Cause Confirmed:
The web-interactives-embed.js script is being loaded twice by HubSpot automatically — once via hs-script-loader and once via an additional injection. This is a platform-level issue that we cannot fix from our side.
Business Impact:
- All popup forms on innovapptive.com are broken
- Visitors cannot submit lead generation forms
- Active campaigns are being affected right now
- Every hour of downtime means lost leads

If this is urgent and still persisting, I’d suggest reaching out to HubSpot Support directly so they can dig into your portal. Based on the portal you’ve included, you should have access to phone support which would likely be your best/ quickest option here.
Based on the context you’ve provided above, I’d suggest checking if the HubSpot tracking code is being injected from more than one source (e.g., hardcoded in your site template AND through a tag manager or CMS plugin).

@SanjayChin - My only contribution here is that pop-up forms/ctas are working as normal on site instances that I manage.
