I have a form on my website. I want to create a report on the form submission, e.g. Names, lifecycle and submission date would be on the report.
Any suggestions on how to do it?

Unintuitively, this is not possible. (Yeah, I know.)
You would have to create a custom date field that is maintained by a workflow and stores the form submission date if you want to build such a report.
For an out of the box solution, it’s better to look at the form submission page directly (Submissions tab on the forms page).
If the submission date is not that important, I’d recommend going with a filtered view on the contact index page (Menu > Contacts > Contacts or, in the new navigation, Menu > CRM > Contacts) and applying a filter for “Form submission” on that page. You can then adjust the columns to your needs and save the view.
It’s not called a report but it’s a lot more workable and usable than a report.
