Difference Between Preview Page and Website Page for custom coded form

Hello,

I have created a custom form within a module and commented on the old module form and added the new module there in both the portal Ids 14486420 and 8163023. In the preview stage of the pages, it’s showing fine but when we check in the existing page links it’s showing a distorted custom form. For clarity, I have added a video link below you can check the same.

I would check to see if some css is bleeding in from somwhere.

Looking at the live page now, it appears that you may have solved this…if so, what was the answer?
