Can't get required field asterisk to show up.
I'm having a hard time figuring out why the required field asterisk will not show up http://info.commerce.ama-assn.org/credentialing/contact-us . I reviewed the CSS for the landing page and removed anything related to that, but it hasn't read more

Form fields not populating labels
