Hello, 

I am having issues segmenting the contacts by the lead form they filled out. I created a list and I unable to edit the column fields to make this display so the sales team knows what form they filled out. Can anyone please help?

Hello @shonn ,

I think if you reach this URL by just adding your portal id - https://app.hubspot.com/analytics/{'Your-HubSpot_portal_id'}/forms

You will reach to section where all form sumbissions data will appear with performances and other insights.

And one more option for you is to create a report of form submissions and I think you will find this report in your hubspot library also if not then you can create one. If you needed any help inbox me I will help you.
