Form with lookup up based on zip code

We sell into the High School market and have all schools in US already loaded as companies within HubSpot.  When a prospect completes form, we have issues with duplicates (Centennial HS vs Centennial High School).  We cannot use email since emails are often associated with the district and not the school.

Ideally, we would like to start form process with zip code, then display all the schools within the zip code as a dropdown/picklist of related schools.

Has anyone accomplished something similar or know of any 3rd party vendors?

Hello @JamesDickinson when you mention lookup zip code would be after the email is submitted you can identify based on the zip code they are using, or offer customers a dropdown menu option with the zip code and depending on their reply show a different form option?

Ideally, we want to use zip code to provide them a list of schools in that area - pulling from our company list ideally.  We cannot rely on emails since most often linked to the school district rather than the specific school.  Once they select the school, we can capture their email and name.
