After form submission user gets redirected to empty page from hubspot forms API

We have an issue with one of our custom forms which on submission we post to HS forms api. After successful post there is no redirect to success page, browser stays on blank page. Here is the form: https://teamhood.com/contacts/

Any ideas how to solve it? Tried double checking everything but seems to be fine. We have not changed anything on this form for past 6 months, and it stopped working couple of months ago...

Its little difficult to diagnose the issue externally. 
You can go through this article here and here . I hope it helps you to resolve the issue.
Still having problem, thn i will advise you to open a ticket with HS support. In your HS acc, navigate to bottam right and click the blue Help button. There you can have a live chat with HS support who can guide you.
