Looking to have Won deals fill out sharepoint form

Hello all, 

I am looking for a way when i have a deals move to "won", a share point form is filled out. all properties pushed from hubspot data.

Do you want to nudge a user to fill out a form or do you want to automatically submit a Sharepoint form with deal information?
