Assigned Contact Notifications & State under properties

Hello everyone,
 
These 2 are not working on my account.  Does someone know how to figure it out?  
 
By the way I already have my settings to receive notifications when a contact is assigned to me.
 
1. I don't get notifications when a contact is assigned to me
 
 
2. The state won't show up for some of my contacts

You can adjust your CRM nofiicaion settings here: How to set up user notifications in the CRM

 

How are contacts being added to the database? Import? Form? A little more context will help troublehsoot further.
