Outlook Desktop 2016, Can't login

Been using the Outlook Sales Extension for weeks, and all of a sudden, it's not active in Outlook.  When I choose "login" from the extension button in the ribbon, it asks for email and password.  When I enter those, it flashes a screen to enter a log in code, but then immediately reverts to the email and password screen.  I then get an email with the log in code.

How do I login to this Sales extension if the screen for the login code doesn't stay long enough to enter the number?
