How can I add Job Title to the list of Properties in the Add Contact Section?

As you can see from the Screen Shot I should be able to make adjustments from the desktop but I cannot figure out where to do so.

After following your guidance I was able to add Job Title to the list of properties in the add contact section on the mobile app. However, I also lost the phone number, contact owner, and lead status section. It's like no matter how many times I make an adjustment in the "Customize The Create Contact Form" and save, the mobile app just doesn't reflect any of the changes. Thanks for posting!
