New contacts are not being added

I have a signup form on my website, whenver any user signs up and enters his data I get a notification and his contact info will be added as a new contact. 

For some reason that stopped happening! Help!?

Have you stopped receiving the email notifications, or have contacts no longer being added to your database? What kind of form are you using on your website? Would you be willing to share a link to your website page where you're using this form?
