How do I exclude specific non-HubSpot forms from being collected by HubSpot

email notifications form

I have always used Hubspot's Forms option and have always received email notifications whenever a user has completed a form. But now, despite all the boxes being checked, I am not receiving the notifications, what can I do to fix this?
