Trying to insert pictures Via Forms for client side submissions

Hi guys,

We are attempting to capture information at site assessment level which will require either contractors or the client themself to submit a form requiring them to take pictures and upload it via our form link 

we have attempted to insert an upload file however the form is telling us the code is unsafe, is there any work around to this ?

For refrence we are a solar company that would need to be having pictures captured for installation and grid application submission.

If yes, could you please share a screenshot of the error message about the unsafe code?
