Troubleshooting Collected Forms

I've checked the following and the form is still not being collected on our Contact Us Page:

• [confirmed is inside form tags] Form not enclosed in form tags 
• [confirmed not in an iframe] Form contained inside an iframe
• [confirmed form has no Javascript bound to the submit] Form has JavaScript bound to form submit event or submit button click event.
• [confirmed that the code loads first by placing it in header, after trying in several different locations on page] Form was introduced after the page completed loading. 
• [confirmed that no sensitive field exists] Form contained a sensitive field, such as credit card number.

Hi @dvfiMelissa  i was able to successfully submit. See my contact record here. Are you able to provide any other information about failed submissions?  IE browser and version number, IP address, other information about the submission?

I'll be happy tolook further with more information.
