Non-hubspot form isn't working

I've added the Hubspot tracking code using Google Tag Manager and have verified that it works. When I test my non-Hubspot form on my site though it doesn't show any submissions and I can't view the non-Hubspot form in the Forms dashboard. Can someone help me troubleshoot this? Form is located at www.predict5.com/purchase

It looks like you are using React JS and I see a script error in the web dev tools console. If you are not the developer, I would share this screenshot with them.

First thing first, please make sure you have checked our compatibility requirements here: Use non-HubSpot forms

Second, when I try to navigate to the URL you mentioned I get a blank page, could you verify this again please? Are you getting a blank page?
