How can we see when a form was deleted and which user deleted it?

This information can't be accessed directly. You'd have to reach out ot HubSpot technical support (via the purple in-app help widget in the lower right corner of the app) and ask them to investigate. (In the past I've found that they usually answers these kinds of questions.)
