How can I add an image to a form that then acts as the sharing-image on Linkedin?

The image should be added to the page that the form resides on. The protocol is called Open Graph ( https://ogp.me ).  Once you add your code you can test the page via the Facebook Sharing Debugger ( https://developers.facebook.com/tools/debug/ ) or Linkedin Post Inspector  ( https://www.linkedin.com/post-inspector/ ).  Then you can confirm how your page will look once shared.
