If you're able to share the link to the page that's causing the issue, I'd be happy to take a look and see what might be causing the issue. At first glance, a couple of thoughts:
1. If you're using WordPress, you may want to check out the HubSpot plugin for WordPress. Once installed, you should be able to add a form to a page using simple shortcode.
2. Based on the screenshot, it looks like the form embed code may not be properly placed within the HTML on your site. Can you try opening your WordPress site's HTML editor and repasting the form code within <div> PASTE THE EXPORT CODE HERE </div> tags and see if that fixes it?
