Collected Form Not Grabbing Contacts

We're having issues with the new Collected Forms tool. It works perfect for our generic Contact Form 7 forms, but doesn't pull leads from our custom forms. 

I've touched base with our webdev but he wasn't able to shed any light on the reason.

Hey @maxconrad, 

Have you checked out the documentation that HubSpot has for collected forms? They walk through troubleshooting your collected form, as well as why your form may not populate in the collected forms tool.
