It's a simple form: three options, no user data input, just a clean and simple satisfaction collector.
Please make this available as a template slide-in in HS.
There are numerous vendors on the marketplace with star rating forms, or we could make the slide-in ugly with checkboxes and force email submission, but this exact form is what we'd like to see.
