Hide Email if known on forms

On Forms, if a cookie is known, provide the option to hide (or gray out and disable) the email so users cannot modify it. If the cookie is unknown, allow the user to type in their email.

Even better, if the membership cookie / user email is known, provide the option to fill in the form with this email information.

Critical Issue:

We have clients who sign into our portal (with cookies enabled) and we have to display the email address on the form because the form does not know who they are yet. This creates the opportunity for the client to use a completely different email address, and creates all sorts of issues regarding the clients data and progress.
