How to set up form with multiple form fields for name and phone number?

Hi all,

 

Happy New Year!

 

I'm trying to create a HubSpot form which captures two people's information, the referring party, as well as the referred party. I want the form to have the following fields:

  • Your first name
  • Your surname
  • Your phone number
  • Your email address
  • Your friend's first name
  • Your friend's surname
  • Your friend's phone number

Could you please review and let me know how I can set up a form like this?

Hi Maret, 

 

Each form can only have one instance of each property at a time - and can only create or update one contact. 


This means that either your referrer or the referred party's details would have to go in custom properties, while the other uses the native contact properties. It would then be a manual process to do something with these details. 

 

Depending on what you plan to do with the referred contact details, I suggest you be wary about GDPR and obtaining their consent.
