Re-subscribe not working?

Has anyone else run into issues of contacts re-subscribing to email lists, only to notice that the contact still shows up as "unsubscribed" the next time you go to send them an email? We've noticed that happening several times within our organization (meaning current employees) so I wonder how often it is happening with contacts outside of our business.

My first question is: do you youse double-opt in? I want to rule that out as a potential factor.

 

Secondly, how are contacts resubscribing to lists and are they choosing to resubscribe to ALL emails from your business or only certain email types?
