Is there an Ad Campaign Report with all these fields?

I'm trying to get a report that shows me all of the fields below. So far, I've only been able to find a few of the fields in a custom report and a couple of the fields in the canned report called "Ad campaigns with cost per click."

I'd like the following:

Campaign

Network

Spend

Impressions

Clicks

CTR%

CPM

CPC

After 1 year there are pherhaps news or new implementations? I'm too still having this problem.
