Setting Survey at webpage

Hello guys,
I'm having troubles trying to add survey at a application.
Apparently the settings is correct, but at the page nothing shows up.
And when a try to run hsFeedback.loadSurvey() manually, got this: 

Idk why neither what could been undefined, and if i access the link, got this: 

Any tip?

Hi @PamCotton, thank you for the support, somebody messed up with the settings, how i hadn`t access to confirm that everything was ok before i come here, couldn`t solve before, but it worked now after they correct the settings on creating the survey, on the front-end the configuration was fin
