NPS Survey is popping up within 3 month frequency cap

Our NPS survey has been popping up for some of our users more than the frequency cap is set (3 months). Trying to figure out what the issue might be - can see a cookie dropping on my own browser if i clear my browser history, but our internal customer service team has been seeing it pop up multiple times during demos.
Has anyone had this issue recently? Hubspot CS hasn't been helpful in finding a technical solution...
