Embedding a customer support response/ survey on a Shopify website

I am looking to set up a form on my Shopify website that allows me to list a filter/ set of specific product/ customer support issues. The aim is to then classify all the incoming tickets under these issue buckets in order to track them. 

 

Looking for some help on how to get this done. Thanks in advance!

Here's what I think you're trying to accomplish, please give a bit more detail if this isn't correct:

 

1 - Add a form to your site

2 - Use that form to trigger ticket creation under various categories

 

You could do this via a combination of a form from marketing module and a workflow that creates a ticket:
