Issues with custom quote Approval and PDF download

Our quotes look great digitally, but when we download them as PDFs, they are not rendering correctly. We looked into it and don't seem to have a way to correct this -- any suggestions?

In addition, it looks like the "approve" button isn't working on the custom quote.

What fixes have you guys found for these?
