Short A: Notes cannot be added via a HubSpot form.

Longer A:

You may use the HubSpot default contact property 'Message' instead.

Message field works, but not well.   Form messages seem to disappear into the depths of contact info, rather than become part of the conversation that a sales reps looks at.  Messages should appear as recent activity.
