Forms not adding information

We have set up a form and are using the link in a browser for our walkin log.  I am receiving email for the form BUT the information is not being added.

 

Any ideas?

As noted in this article you can run into problems when submitting forms multiple times from one device as HubSpot defaults to using coookies. There is a new option called "Always create contact for new email addressses." which was implimented to solve this exact problem.  You'll find it towards the bottom of the options tab in the form creator.

@ConnorSlivensky Thanks. I believe the actual issue is all the personalization fields created a new field inside of HubSpot properties.  How does this get adjusted?
