How can I eliminate fields in this form that show up automatically -- specifically company and contact?

Head over to Settings > Deals and click Manage next to 'Set the properties your team sees when creating deals.'

Thanks Phil.  But I don't see "Set the properties your team sees when creating deals" anywhere on the settings>deal page.  I am using the free version right now.  Is that the reason?

The option appears to be called 'Set the default deal properties displayed in your portal' in free portals. 

You should be able to do what you need in there.

Nope.  Can add additional properties, but cannot delete the ones they insert automatically.

You're right, I got my wires crossed somewhere between all the portals I have open. 

Seems this feature becomes available when having a paid marketing subscription.
