City Name are missing from Contact Board

In Main Contact Board name of the cities are not reflecting Only for those contacts who are submiting the form on my website.

Want to confirm that I am reading the question correctly.

The city is not displaying on the contact screen for anyone who submits a form. 

Follow up questions:  

Does the form ask for a city?

Does the city appear when you look at an individual contact?

Did the contact have a city already associated with it before the form was submitted?

Is the form hosted on a HubSpot site or elsewhere (such as Wordpress)
