Best process for agent to do a questionnaire for each lead.SOLVE

Our company gets a lot of leads, our agents call those leads and they perform a questionnaire to make sure the lead qualifies for the next step. They ask series of 10 - 20 questions, some are logic based.
Ex. If lead says yes to question 2, then they jump to question 5 etc.
Management wants an easy way to see the answer to those questions on each lead as soon as they open the lead.
Is it possible to add a tab t other main page of the lead? What other recommendations do you guys have?
I was hoping there was a way to add a tab, and have all the info there.
