Quote Template Header and Footer

Hi I have added certain code to show up header and footer in my custom template . 
While generating pdf , header and footer not printing on each page. it is fixed at top and bottom end of entire page.
Is there any option to make that repetitve in each quote PDF

I have ran into the same issue. Even setting the footer as fixed did not work. It seems the PDF generated looks at the page as one long document and breaks it up in A3 page sizes.
