Forms with audio recording capabilities

Hi,
Instead of typing text into a field of a form, I would like to let the user record a short audio that is added to the form as a file.
Has anyone tried this?

• Will you be using a custom-coded form and the Forms API?
• How are you planning on handling the recording functionality? 
  • Will you use something like the MediaRecorder API, for example?

Thanks for getting back to me.
1) I thought it would be difficult to add an audio field/controls within an existing form, so I was planning to copy the content of an existing form into a new module. I don't know if this is what you call a 'custom coded form'.
2) I started using RecordRTC.js but ran into some difficulties when trying to save the audio as a file before adding it to a field.
