Form on Modal not capturing data in Hubspot

I have a problem on the following web page:
https://www.tarasafe.com/chief-fr-coverall.html

When we click the CTA Button (Request a Sample) a modal gets opened which loads an external php form.

The data from this form is not getting recorded on hubspot.

I am using a similar form on another page where it is directly embedded on the page itself:
https://www.tarasafe.com/enquiry.html

I don't want to embed the form directly on the page as it is being used in 50 pages.

Can somebody help?

I think we will need to see the php that is responsible for the submission to help out here.
