Is there any option for this type of customization or do we need to look into our own form development and then submit via API?

Sorry to be the bearer of bad news, but this doesn't exist to my knowledge.  I did find a few examples on codepen that could be of use.  You would need to do custom form development for this one then use the forms API to submit to.
