Custom Module Lightbox with a form

Hello Guys,

I created a custom module for a popup. But i need create a custom module where the user select a form for the page. (similar to module form) and i dont have ideia for how I start.

Actually my custom module have rich text and text, the text for button and the rich text for text or images, and this appears only in a lightbox.

How i make this?

You can use HUBL to add a form module to your custom module. See docs here http://designers.hubspot.com/docs/hubl/hubl-supported-modules#form
