The warning message says that emails will be sent to contacts who have been granted access to the private content (through access groups) but have not yet registered. If the contact had received the registration email in the past, HubSpot will not send the email to the contact.
There hasn't been a change in the behavior of private content emails but the warning message is relatively new in the UI (end 2024) The intention behind this change is to inform users about the expected consequences of publishing/editing private content because the most frequent pain point of using memberships is 'accidental' email sends where customers are unaware of when their actions might cause registration emails to go out.
So by providing in-app consents (i.e. the warning message and checkbox), we can effectively warn users when they publish private content that X number of contacts who have access to this page have not yet registered and that emails may go out to these contacts.
So what you should do is to review the contacts in their access groups. You can check if these contacts have received a registration email in the past by referring to the private content access properties.
If a contact has received a registration email for the domain that the private content is published to, the "Time registration email was sent" property will be populated with a value. Contacts without a value in this property are those who have not received a registration email.
