The form mentioned is at the bottom of Log in to HubSpot

 

It will appear when you click 'If you're unable to log in after following the steps above, click here to submit your information.'

Hi, 

Thanks for the reply.

However, the link referred to a form which was not there.

I previously had an account under info@envirovaluation.org but I cannot seem to get it back.  In the past I was deactivated and recovery was easy.
