Need help in submitting Design Practicum

Hi,

I have completed my Design Practicum but I am not sure how to submit "Practicum Template URL" and "Practicum Live Page URL" as I made my page in Design Manager but not able to generate template URL to submit in the form. Also, for live URL submission do I need to purchase a domain and hosting?

Please help!
