Google Analytics<> Hubspot Integration

I am trying to implement the closed loop GA<>Hubspot integration as is outlined here:

I'm stumped at step 5 with the form integration section. I dont see these options in editing forms:

Is this article also outdated? Does anyone at Hubspot actually have an article on how to do this? Seems like a total no-brainer basic integration to me......
