I would like to customize the design of the survey email and use our own email template or template modules. An example would be to be able to create a custom NPS score table within a module.

HubSpot offers a Design Manager that can be used to create custom modules as per youir design requirements - https://knowledge.hubspot.com/cos-general/get-started-with-the-design-manager

I can build the custom email module for you. Once designed, you can use it multiple number of times.
