Take a look here, Resubscribe contacts to your marketing emails, under the header " Resubscribe a contact through a HubSpot form (Marketing Hub Professional, Enterprise, or Legacy Marketing Hub Basic only)". you can add this to your forms so they just simply go here and that will get that popup.
