I would like to analyze the performance of a specific form and compare “the date ‘specific’ form submit” vs " the date contact enter lifecycle stage won date". I am going to develop a pivot table report from custom reports. I could add “the date contact enter lifecycle stage won date” but how can I add the specific form submit date into that report?

Unless you are actively maintaining such a form submission date (e.g. through a custom date property anda workflow), you currently can’t. There is not form submission data source in the custom report builder, which means that there is no out of the box way to see in the custom report builder how many submissions were made or when they were made. The contact data source does not contain this information.
